Community Support Fund Grant Application
OLBH supports its community through strategic grants. All requests for financial support must be made in writing and should be made on the application that can be downloaded to the right. In addition, the W-9 at right must be filled out and submitted with all grant requests. Telephone solicitations will not be considered.
Contributions are almost exclusively reserved for the organizations that directly benefit OLBH’s service area and are made principally in the areas of health and human services, civic affairs, education, quality of life and the economic development.
Guidelines
- OLBH does not make contributions to individuals, or to causes that benefit a small number of individuals.
- Funding request priority is based on the overall benefit to the community.
- In the areas of education, preference is given in the fields of healthcare and science.
- Organizations receiving contributions must be in compliance with the Equal Employment Opportunities Act and other federal, state and local laws and regulations.
- In general, contributions will not exceed $5,000.
- Requests for $500 or greater requires the Community Support Fund Application be completed.
- For requests for less than $500, a letter can be submitted with all details.
- A completed W-9 must be submitted with all contribution requests before the request will be considered.
Our Decision Making Process
A community support committee reviews all donation requests monthly. If a request does not meet established criteria, you will be notified soon after the team’s review.
The committee reserves the right to change the amount of your request, ask for additional information and ask for a formal presentation.
